I developed this application with a variety of thought provoking questions designed to promote exploration about what you really want out of life and for your health. Please set aside 15-30 minutes to thoroughly consider these questions. There may be some questions you are hesitant to answer, thus, if you feel uncomfortable answering some of them, you may leave them blank. However, please know that as your health coach, it is important for me to understand who you are, what you value, and how you approach life so I can better serve you.
